ANGELS for ANIMALS
DOG SURRENDER FORM
Please read the below information before completing this form.
**If you are needing to return a dog that you adopted from Angels for Animals, you do NOT need to fill out this form. Please call 330-549-1111 and ask to speak to one of the Dog Managers. We accept any of our adopted dogs back at any time.**
Following is information regarding our dog intake policies. SUBMITTING THIS FORM DOES NOT AUTOMATICALLY PUT YOUR DOG ON THE "WAITING LIST" - PLEASE READ THE POLICIES BELOW CAREFULLY!
Angels for Animals does not euthanize animals to make room for other animals. Adoptable animals are at the shelter until they find a permanent home or move into a foster home, which may be a matter of weeks or even months. Therefore, we are almost always "full" and space for incoming dogs is limited. Because of these factors, we are seldom able to admit dogs on a walk-in basis or with short notice. In general, we are more often able to help people who have given themselves time to find their dog a new home than those who need to "get rid" of the dog today. We understand it is important to you to place your dog into a facility that does not euthanize for space. However, if you are unable to wait for available space, please know that there are other options.
Some dogs, seniors in particular, often do poorly in shelter settings. These animals can decline quickly in health and behavior in a shelter environment. They are usually the last to be considered for adoption, making their shelter stay longer and continuing the decline. We ask that people seriously consider the negative effects that a shelter environment can have. Our Dog Manager will be happy to discuss these issues and other options with you.
The first step in the surrender process is to complete this form. Please answer all questions and with as much detail as possible. Our Dog Manager will review your information and determine if your dog is appropriate for our adoption program. Please understand we receive MANY requests to take in dogs every day and are unable to accomodate everyone. Be sure and provide us with a working email address as we may contact you by email.
If the Dog Manager believes your dog may be a candidate for our adoption program, you will be contacted to schedule a Surrender Appointment. At that time, you will bring the dog to Angels for Animals along with any veterinary records. The Dog Manager will evaluate your dog's behavior and our medical staff will heartworm test the dog. The dog must be both heartworm negative and behaviorally adoptable for us to accept him into our program. All intake decisions are at the sole discretion of the Dog Managers.
We ask for a donation of $75 help offset the costs associated with vetting and caring for the dog while at the shelter. It costs us several hundred dollars to get a dog ready to be adopted and nearly $10 PER DAY to care for each dog here at the shelter. If you are unable to make that donation, please still fill out this form and we will try to work with you. Our first concern is the dog!
If we have space available, we will take the dog in. If we do not have space your dog will place on the waiting list, you will be advised of the approximate entry date into the shelter, and you will be asked to keep your dog until a kennel is available.
As stated above, not all dogs can be accepted into Angels for Animals shelter. If your dog does not pass our behavior evaluation or is heartworm positive, we cannot take your dog and we will suggest other options such as private placement, breed rescue, or the county dog pound.
